ONLY PLAYERS WHO WISH TO PLAY ON A 3 ON 3 TEAM NEED TO FILL OUT THIS REGISTRATION FORM.

						All paticipants who are playing in the 3 on 3 tournament will receive a jersey upon check-in. We request jerseys be returned after the event. A keep-sake event T-shirt will be given to all players and carnival participants. PLease list sizes below
